Proposals Due 3/2: Medical Assistance Program Vision Screening and Treatment Grants featured image

Vitalyst is currently seeking proposals for vision screening and treatment programs serving low-income families. Grant amounts could generally range up to $80,000 annually, for a three-year award period (July 2018-June 2021).

Funds may be used for direct services (such as screening, training, surgeries), program development related to direct services, and client-related equipment (such as regular and specialty glasses, assistive devices and adaptive equipment).

Vitalyst’s Medical Assistance Program continues the legacy of donor-directed endowments that began with the St. Luke’s Health System – from which Vitalyst Health Foundation was created in 1996.

Proposals are due by 5:00pm Arizona Time on March 2, and will be accepted online only. Learn more, and access the online submission form here.
