2014 Innovation and Medical Assistance Grant Funded Partners

Innovation Grants

St. Luke’s Health Initiatives’ 2014 Innovation Grant funded partners are:

  • Circle the City, receiving $125,000 to expand the Frequent Users Systems Engagement (FUSE I) pilot beyond hospital discharges to emergency services. (FUSE) II proposes to identify, house, and provide support services for frequent users of publicly funded emergency systems (fire, police, human services & courts).
  • Hospice of the Valley, receiving $110,000 to implement and evaluate an innovative training for caregivers using Mindfulness Based Stress Reduction (MBSR). This increases the capacity and resiliency of dementia caregivers while reducing the cost of care.
  • International Rescue Committee, receiving $125,000 to connect local, healthy food production with corner stores through financing/Technical Assistance and a loan pool. This leveraged financing for 25 owners to create healthy food displays in local corner stores while incentivizing 250 low-income shoppers.
  • The Arizona Partnership for Immunization (TAPI), receiving $125,000 to redesign public health & medical care delivery models aligning with ACA guidelines. Additionally, leveraging opportunities to bill AHCCCS and private insurance for community health services. This supports the public health safety net by changing reimbursement policies and enabling sustainability.
