Creating enduring change requires us to focus on the roots of each issue, seek partners who can enrich our perspective and supplement our resources, and propose a plan that will benefit the community. Focusing on the causes of the problem – not only the symptoms – and joining forces with others can magnify the effects of the financial and human resources that are invested.
Innovation begins with a creative idea about improving access, health or well-being for a vulnerable population. Understanding all the facets of the problem requires partners that provide a different line of sight on the problem to more effectively construct an intentional plan of action.
Next, the financial and human resources that are needed to implement the project must be carefully assessed.
Finally, evidence must be gathered about how project results have changed individual outcomes. Evidence typically includes stories of what is working as well as specific, measurable differences in outcomes. Project evidence may lead to ongoing adjustments to the program as continuous learning takes place. The evidence should also make the project sustainable by showing other funders the value of the work being done.
Innovation Grants Goals:
- Increase community capacity to identify and address issues affecting community health, well-being and access to care.
- Strategically leverage infrastructure, systems design and policies to improve health where we live, learn, work, pray and play.
- Improve the environmental, social, behavioral and health infrastructure conditions that impact determinants of health and enable Arizonans to be healthy and resilient.
- Create a sustainable solution to the problem being addressed that endures after the term of the grant funding.
