Arizona’s hospital readmission rate has decreased to 15.7 percent (in comparison to an 18.4 national average), thanks to the efforts of health care professionals statewide. What comes next is work that lies at the intersection of health care and the social determinants of health. Anyone with a stake in increased care quality is encouraged to attend: hospital and clinic leaders; directors of case management, pharmacy and quality; home health agency owners and clinical directors; community health providers such as pharmacies and care homes; and community-based organizations and stakeholders.
