Arizona’s hospital readmission rate has decreased to 15.7 percent (in comparison to an 18.4 national average), thanks to the efforts of health care professionals statewide. What comes next is work that lies at the intersection of health care and the social determinants of health. Join Health Services Advisory Group, Vitalyst Health Foundation and the Federal Reserve of San Francisco on March 27 from 8:00am-4:00pm as we examine strategies that improve care coordination, decrease readmissions and improve health.
